It's pretty clear there is a salary cap by the Milan ownership. And if they want to keep their star players, they need to break that cap. Donnarumma would have broken it, Zlatan broke it (but the contract was one year). Now they need to decide will they break it for Kessie.
If they decide against breaking the cap, it will pinpoint that Milan (their owners) aren't in it for real. Their business is not to win the Scudetto for real. Of course they can win it by chance, but to build towards that, you need to put the money on the table. Losing Kessie would mean they'd be banking on Tonali and kind of "starting over". Tonali can't carry them yet, at least not like Kessie did last season.
