As a lifetime hypochondriac, the doctors sometimes to diagnose use the process "diagnosis of exclusion" or the process of eliminating, I suspect they start by eliminating the most severe things and going downwards.

I assume there are specific things that would be known/symptoms of torn ACL and if it did not have that they'd say it was a sprain..
